- Photocopy of NRIC (front and back) AND
- Either of your: Latest 12 months of CPF contribution history, Latest computerised payslip, OR Latest Income Tax Notice of Assessment
For Commission / Variable Income Earner:
- Photocopy of NRIC (front and back)
- Latest 2 years of Income Tax Notice of Assessment**
For Salaried Employees with Commission above 20%:
- Photocopy of NRIC (front and back)
- Either of your: Latest 12 months of CPF contribution history, the Latest 3 months of computerised payslip, OR the Latest Income Tax Notice of Assessment
Additional documents for foreigners:
- Valid passport
