I got my Citi premier miles card start of this year, and I spent 7.5k in the first 3 months but they never gave me the option of paying annual fee... so how do I get the 30K miles? Or do I only get it after I pay the annual fee once one year is up?

The annual fee occurs only at the end of one year. Therefore, continue to use your card as you normally will.
Once you reach the anniversary and the annual fee is charged, the points will be awarded accordingly. Citibank makes it very easy, so don't worry.

It's an "annual" fee so obviously you'll only be charged when a full year is up ^^; That's how they hold people onto the credit card for a year and beyond or people would just ditch the card if they happen to only need the 30,000 miles for whatever purpose they have.
The annual fee is automatic, by the way. You'll see it in your statement and together, you'll see that 30,000 miles have been awarded to you. As long as you pay your credit card bill and don't call up to get the annual fee waived, the 30,000 miles will retain in your account. If you got confused and called to get the annual fee waive, they will immediately deduct back the 30,000 miles from your account.
