If your mental wellness can still hack it, stay on, take a few days of leave to recharge then decide post surgery given the comp medical benefits would generally cover.

I would say that it depends on how much your current job is affecting your mental health. You will have to sit with yourself and find the root cause of the anxiety and work from there in order to resolve the emotions you are feeling.
Seeing that you are going for surgery in the coming month, I would advise you to stay in your current job until you have fully recovered post-surgery. The reason is, you can take hospitalization leave/MC/Leave during that time and still get paid while you are recuperating. Heck, you can even use this time to rediscover yourself.
Lastly, if you decide to leave your company, make sure to leave when you get accepted for another job. You can also arrange in such a way that you can take a few months off before starting at your new place.
