Hello!!! From my experience, both Citi and AMEX clock miles for hospital bills.
Let me share my Citi PremierMiles experience, I paid for my medical bills of about 7k with Citi PremierMiles, but bad thing is, if you did not declare this at the start upon registration for the card (that you want to pay annual fee), they will not give you those bonus miles!!!!
It's really horrible cause after calling them they told me I cant get the bonus miles even if i wanted to pay for annual fee. I feel like I wasted my miles on this card! You can read an indepth description of my experience here: https://seedly.sg/reviews/credit-cards/citi-pre...
On the other hand, I had to pay for hospital bills a second time, and I signed up for an upgrade to AMEX ascend from my Krisflyer. This time, it was super fuss free, I got my 36k miles and their customer service was stellar.
Can't advice you much on cashback but I think if there's first time sign up + bonus miles, miles confirm more worth it!
