Does anyone have experience that HDB has rejected for grants and hdb loan as they have included bonus when calculating income ceiling? is this a mistake from hdb officer?

You ask a good question. Originally, I would think the "average gross monthly household income" include CPF contributions, bonus and AWS https://stats.mom.gov.sg/SL/Pages/Gross-Monthly...
Yet the others are right to point out that based on HDB website that the assessment of household income and credit assessment does not include bonus.

I have experienced rejection as well and it seems to suggest that the calculation has included bonus thou. Hope someone can share their personal experience.
I am now writing to HDB to seek some clarification.
