Yes, I have done mine after I got into a horrific car accident a couple of years ago.

Accordingly, we will sign the prescibed form in the precense of two witnesses at the same time. Moreover, one of the witness shall be a medical practitioner and the other shall be a person above 21 years old, and has no interest in any of the moveable or immovable asset that we own.
Other than that, the process is pretty much the same. Oh yes, not all doctors are willing or able to help you sign the directive. Hence, do take note of this if you intend to do one.
