More than 5 years ago, I went for a medical check up for irregular period and it was concluded that I was low in iron. Doctor only gave me iron supplements and there was no other follow up . Do I still need to declare this since I was not diagnosis with anything?

In the interests of disclosure, you should declare this if there is question relating to being on medication, without any specific timeframe. This really depends on how the question is worded, so please read the application carefully.
Remember that an application for insurance coverage is one made in utmost good faith.

Mm I'd say, follow the health questions required on the application form and just declare accordingly.
Some insurers may require medical examination/blood/urine tests; this is dependent on a couple of things. Your age / sum assured you are applying for and your current health status. Medical requirements will differ from insurer to insurer.
