For aviva and GE, the payout excludes the base careshield payout if you suffer from not being able to perform 2 ADL. if 3 ADL, on top of your supplement payout, the base careshield will pay out as well.

for ntuc income, if your supplement payout amount is $5000, it will remain as $5000 regardless of 2 ADL or 3 ADL.

Depending on the plan you take, NTUC's payout does include the base Careshield payout, where as Aviva/GE does not.

Yes, the payout can be above $5000/mth (plus careshield payout) if you choose the escalating payout option, but note that your premium will escalate as well.

If you're going for escalating payout option, cap your premium term (i.e. limited payment term) to control your insurance costs in retirement.

If you are going for a level payout, a premium term till 99 would still be ok; either it will always be below the Medisave limit of $600/yr, or you will just pay a bit of cash every year (just like how you would pay cash with the rider of an integrated shield plan as long as you live)
