No. The limit of $600/yr is per insured for supplementary plans only. So you are correct to say that it is $600 + X.

If you split the careshield supplement planpremium across two persons, it must still add up to $600/yr (e.g. a husband could use $300/yr and his spouse, a housewife, could use $300/yr, to get the supplement plan for the spouse).

Please note the GST issue with careshield supplements as well. If GST goes up, and your supplement plan costs $599/yr and will now cost (say) $605/yr, that $5/yr must be paid in cash/GIRO. To have the plan lapse over $5 would be a waste.
