Hi anon,
This depends on what you are claiming to be frank. I've never had much issues claiming my GP medical bills from my group insurance when I used to work in corporate world. It just took time, but I wasn't in any rush.
Probably you are referring to the MHA insurance. For payouts more than $200K, it's $200K for first payout and the subsequent amounts split equally over next 3 years, but that is just a feature of the plan and doesn't affect the ease of the claim.
Frankly, if the payout criteria is met, it's almost impossible to deny the claim. Straight forward claims such as death cannot be disputed (Just produce the death cert), but limitations on the plan may affect the payout amount or structure.
Make sure you have your own personal coverage. Group insurance, while helpful, should not be your main protection plan. You can change companies, or your company may decide to reduce benefits, etc.

It depends on the type of benefit that you are attempting to claim. Generally, all insurers issue a payout so long as the conditions are fulfiled. Unless there is a specific question or condition, otherwise, most insurers work the same way.
