How do I fill up the W-8BEN form from Singapore to invest in US stocks?

I recently tried logging in through my Standard chartered online banking account but was denied the option to invest in US stocks as I had to fill up this form. I was abit confused so I was hoping someone can share with me how to fill this up?

Oh! I literally filled this up a few weeks back. Anyway, yes the terms look really confusing and complicated.

But actually you will only need to fill up the top and bottom parts for Singaporean citizens/PR.

I have attached a screenshot attached of a mock form filled up!

You can go down to the bank and get them to assist you step by step. They will tell you which area to write and how to write it. Usually must write in capitals and address must have certain format.
