Ask different insurers for different plans.
ask the insurer what exactly do they need so that there is no exclusion. tell insurer to be as specific as possible: doctor's letter? hospital memo? specialist letter? what needs to be stated in letter? which doctors/ doctor panel are recgonised by insurer? what tests need to be done?
go to the doctor for consultation and tests. tell the doctor you have such condition and insurer's requirements, and how they want the letter to be worded.
wait for doctor's test result and letter and submit to insurer. remember to scan or keep a copy.

background: few years back i wanted to remove an exclusion and the particular insurer's customer serivce and agents (multiple agents) made me jumped through hoops and red tape. even with a goverment hospital specialist (at my own costs), they refuse to accept the specialist's letter and test report.
that is why i suggested for you to ask specifically what the insurer wants to see in the letter.
