Is the saying "higher risk, higher returns" always true? Or are there scenarios where there's lower risk. but still high returns?

What are some scenarios whereby the latter is true? How do you go about deciding whether or not the risk is worth taking?

On average: higher risks = potential for higher returns

sometimes it is risky to not take a risk, an sometimes seemingly 'low risk' indeed is high risk

Lower risk by proper diversification, but also not overly diversify.
