Usually before the patient is admitted to hospital / prepared for surgery - there will be a financial counselling process to estimate the cost of surgery (range) and stay and the kind of ward the person will be staying / how to pay (insurance, medisave, medisave life etc.).

It's on a case-to-case basis based on the condition which might differ from ppl to ppl.

If the surgery is compulsory and necessary -
That said - What you have asked shld not be the question / reason to decide whether to do the surgery/procedure. Factually you won't know the expenses (medicine/tests/procedures) and whether they qualify for claim until the episode is over.
About payment / affordability - the govt hospital does allow payment in instalments / have Medifund / other schemes to help out - you can request to talk to a social worker before the procedure if you are concerned and want to prepare for the cash required.

Medishield Life is applied at the end of surgery / ward stay etc - you/your family can constantly check w the hospital finance/bill payment to check the amount of cash you have to fork out/agree on a payment plan.

Most of the time, Medishield life covers a large portion of the bill (based on real experiences) just that most ppl who benefitted didn't share. Hope this helps.
