You can consider looking at Standard Chartered Unlimited Cashback card. If the hospital accepts AMEX, can try AMEX True Cashback too.

Hospitals can be tricky when it comes to credit card rewards. Some hospitals code under MCC 9399 (government services) which generally does not earn rewards. There should be no issues if it is a private hospital.
Cashback card options are pretty limited. I assume that your bill will run into the thousands, and that you will have to pay it in one lump sum. Citi Cash Back+ offers 1.6% while Standard Chartered Unlimited offers 1.5%. If you are a new cardholder, there may be signup promotions as well. However, they WILL NOT award cash back if it is coded under MCC 9399.
Amex True Cashback is a safe option but not all hospitals accept Amex. You should avoid Bank of China and DBS cards as they explicitly exclude hospitals from earning rewards.
