What is My Legacy?

My Legacy is a government initiative which provides information and services for planning ahead for palliative care and post-death matters on a secured digital platform. It is convenient as it simplifies the processes and documents that would previously have been difficult to locate and follow-through.

My Legacy’s useful features

1) Step-by-step guides

A good consolidation of information across various end-of life-topics such as how to plan ahead, preparing financial and legal documents or even considerations to your healthcare preferences. This information has also been translated in the 4 common languages: English, Chinese, Malay and Tamil. Comprehensive guides on making your Lasting Power of Attorney (LPA) and Advance Care Plan (ACP) are also available to help you understand the requirements and processes better.

Lasting Power of Attorney (LPA)

A Lasting Power of Attorney is a legal binding document that allows us to appoint a Donee to make decisions on our behalf if we were to ever lose our mental capacity.

Advance Care Planning (ACP)

An Advance Care Plan allows us to decide on how we would want to be cared for if we become incapable of making those decisions. After all, the onus is on us to make such decisions and not leave it to our families who would have to grapple with such sensitive and painful issues.

2) Services Directory

Through the Services Directory, you can easily search for a registered Certificate Issuer (either a practicing lawyer, psychiatrist or accredited doctor) to make your LPA, as well as a certified ACP Facilitator to complete their ACP. Those navigating the loss of a loved one can also find out more about the immediate next steps such as registering the death and booking a wake space with Town Councils.

3) LPA-ACP Tool

The LPA-ACP tool allows us to simultaneously fill both the LPA and ACP forms at one go, through a guided and simplified process. This saves time and makes it easier to complete and prepare both forms for submission.

4) My Legacy Vault

With the My Legacy Vault, you can now easily store digital copies of your important documents (ACP, LPA, Funeral Plans etc) in one place. You can also access updated status of your CPF Nomination, giving you a consolidated view of all your important documents and decisions. Apart from storage, the Vault also allows users to register loved ones as Trusted Persons to communicate these plans securely and timely. The My Legacy Vault is also protected with Singpass and Two-Factor Authentication (2FA), ensuring that any documents uploaded are safe and secured.
